Request a Provider ReferralWhat a 329 card means
Under Hawaiʻi’s medical cannabis program (HRS Chapter 329, Part IX), a registered patient may:
- Keep an adequate supply of up to 10 plants and 4 ounces
- Cultivate at one registered grow site listed on the card
- Purchase from Hawaiʻi’s licensed medical cannabis dispensaries
- Designate one primary caregiver
Oʻahu cardholders can also request membership in the Aloha Cultivation community — education, community connections, and consultations.
The process
- See a certifying provider — a physician or APRN in Hawaiʻi. State law allows in-person or telehealth visits. The provider we refer to starts with an in-person first visit; ask them about telehealth for follow-ups and renewals.
- Your provider completes an electronic certification if you qualify.
- Apply online through the DOH registry with your email and a valid ID (driver license, state ID, or passport).
- Pay the DOH registration fee.
- DOH reviews your application, typically within 3–5 business days, and emails you when your card is ready to download.
What it costs
| Item | Cost |
|---|---|
| DOH registration, 1 year | $36.05 ($35.00 + $1.05 admin fee) |
| DOH registration, 3 years (eligible renewals) | $108.15 ($105.00 + $3.15 admin fee) |
| Provider certification, 1 year | Up to $105 (state cap: 3× the DOH fee) |
Providers share their certification fee before your visit. DOH shows your final registration amount at checkout.
Renewals
- Renew up to 60 days before your card expires — early renewal keeps you covered.
- Renewing with the same provider for a chronic condition can qualify you for a longer registration.
- Your new card activates automatically the day your current card expires, so keep using your current card until then.
