Getting or Renewing a 329 Card by Telehealth
Act 241 (2025) removed the requirement for an in-person visit before medical cannabis certification. Since September 18, 2025, Hawaiʻi patients can be certified — or recertified — through telehealth.
Updated October 26, 2026
What the visit looks like
- The certifying physician or APRN is physically located in Hawaiʻi during the appointment.
- The visit meets the same standard of care as an in-person visit, with a documented history and discussion of your symptoms.
- It’s a real conversation with a provider — DOH states that an online questionnaire alone doesn’t count.
- The provider takes on ongoing responsibility for your care, including follow-up.
Who qualifies
Act 241 describes qualifying patients as those with a debilitating medical condition certified by a physician or APRN, those with other conditions certified by their primary treating provider, and those in hospice care certified by a hospice provider.
How long a certification lasts
Certifications are generally valid for one year. For chronic conditions, DOH may allow up to three years.
Where to start
DOH lists certifying providers and the patient application on its Medical Cannabis Registry site.
